Trade aggressively before level 6, especially right after his E misses — it's Swain's only lockdown tool, and it has a long cooldown early, so he has little answer to follow-up aggression.

Disengage the moment he activates his R (Demonic Ascension) unless you can burst him down immediately — he heals himself for every nearby enemy he drains every half-second, so the fight only gets worse for you the longer it continues.

Spread out in team fights and around objectives — grouping up lets a single E land on and pull multiple people together, and keeps everyone inside his R's draining radius at once.
